David Hildenbrand 3f10341ffb s390x: add function to deliver restart irqs
This patch adds a helper function to deliver restart irqs. To be able to be used
by kvm, the psw load/store methods have to perform special cc-code handling only
when running with tcg.

Thomas Huth e3e09d87c6 s390x/mmu: Skip exceptions properly when translating addresses for debug
When a fault occurs during the MMU lookup in s390_cpu_get_phys_page_debug(),
the trigger_page_fault() function writes the translation exception code
into the lowcore - something you would not expect during a memory access
by the debugger. Ease this problem by adding an additional parameter to
mmu_translate() which can be used to specify whether a program check and
the translation exception code should be injected or not.

logging! It's not very handy.

Adding separate log category for *_cpu_handle_mmu_fault()
logging fixes the problem.

zhanghailiang 1a71992376 target-s390x: fix possible out of bounds read
Array index starts at 0, so the valid index of ext_queue array,
io_queue array, mchk_queue array should be MAX_EXT_QUEUE - 1,
MAX_IO_QUEUE - 1, MAX_MCHK_QUEUE - 1.

The original checks missed the invalid bound value, which will lead
possible out of bounds read in the follow codes.

David Hildenbrand eb24f7c689 s390x/kvm: proper use of the cpu states OPERATING and STOPPED
This patch makes sure that halting a cpu and stopping a cpu are two different
things. Stopping a cpu will also set the cpu halted - this is needed for common
infrastructure to work (note that the stop and stopped flag cannot be used for
our purpose because they are already used by other mechanisms).

A cpu can be halted ("waiting") when it is operating. If interrupts are
disabled, this is called a "disabled wait", as it can't be woken up anymore. A
stopped cpu is treated like a "disabled wait" cpu, but in order to prepare for a
proper cpu state synchronization with the kvm part, we need to track the real
logical state of a cpu.

Paolo Bonzini f08b617018 softmmu: introduce cpu_ldst.h
This will collect all load and store helpers soon.  For now
it is just a replacement for softmmu_exec.h, which this patch
stops including directly, but we also include it where this will
be necessary in order to simplify the next patch.

Thomas Huth c4400206d4 s390x/helper: Added format control bit to MMU translation
With the EDAT-1 facility, the MMU translation can stop at the
segment table already, pointing to a 1 MB block. And while we're
at it, move the page table entry handling to a separate function,
too, as suggested by Alexander Graf.

Thomas Huth 422f32c5b1 s390x/helper: Fixed real-to-absolute address translation
The real-to-absolute address translation in mmu_translate() was
missing the second part for translating the page at the prefix
address back to the 0 page. And while we're at it, also moved the
code into a separate helper function since this might come in
handy for other parts of the code, too.

Cornelia Huck 91b0a8f334 s390: Fix handling of iscs.
There are two ways to express an interruption subclass:
- As a bitmask, as used in cr6.
- As a number, as used in the I/O interruption word.

Unfortunately, we have treated to I/O interruption word as if it
contained the bitmask as well, which went unnoticed so far as
- (queued-for-next) kvm made the same mistake, and
- Linux guest kernels don't check the isc value in the I/O interruption
  word for subchannel interrupts.

Make sure that we treat the I/O interruption word correctly.

Cornelia Huck bd9a8d852c s390: Keep I/O interrupts enabled for all iscs.
do_io_interrupt() would stop scanning further iscs if it found
an I/O interrupt it could inject. This might cause the pending
interrupt indication for I/O interrupts to be reset although there
might be queued I/O interrupts for subsequent iscs.

Fix this by reordering the logic: Inject the I/O interrupt immediately
and continue searching all iscs for queued interrupts.

Andreas Färber f94667336e target-s390x: Clean up cpu_inject_*() signatures
Despite cautioning that S390CPU is needed for upcoming CPUState
refactorings, commit 5d69c547d9 (s390:
I/O interrupt and machine check injection.) added functions
cpu_inject_io() and cpu_inject_crw_mchk() with CPUS390XState argument,
claiming consistency with cpu_inject_ext().

This complicates making cpu_interrupt() take a CPUState even more and it
required to pass &cpu->env from some S390CPU-aware call sites already,
creating inconsistency elsewhere. Address that.

This also eliminates the need for CPUS390XState in s390_virtio_irq().

Make the user path more like the system path.  Prepare for more kinds
of runtime exceptions.  Rename ILC to ILEN to make it clear that we
want to pass around a full instruction length, rather than a "code"
that happens to be stored one bit left in a larger field.

Avi Kivity a8170e5e97 Rename target_phys_addr_t to hwaddr
target_phys_addr_t is unwieldly, violates the C standard (_t suffixes are
reserved) and its purpose doesn't match the name (most target_phys_addr_t
addresses are not target specific).  Replace it with a finger-friendly,
standards conformant hwaddr.

Outstanding patchsets can be fixed up with the command

  git rebase -i --exec 'find -name "*.[ch]"
                        | xargs s/target_phys_addr_t/hwaddr/g' origin

Andreas Färber b7e516ce04 Kill off cpu_state_reset()
In commit 1bba0dc932 cpu_reset()
was renamed to cpu_state_reset(), to allow introducing a new cpu_reset()
that would operate on QOM objects.

All callers have been updated except for one in target-mips, so drop all
implementations except for the one in target-mips and move the
declaration there until MIPSCPU reset can be fully QOM'ified.
