For one thing, this allows us to drop the error message generation from
qemu-img.c and blockdev.c and instead have it unified in
bdrv_truncate().

The block layer takes care of removing the bs->file child if the block
driver's bdrv_open()/bdrv_file_open() implementation fails. The block
driver therefore does not need to do so, and indeed should not unless it
sets bs->file to NULL afterwards -- because if this is not done, the
bdrv_unref_child() in bdrv_open_inherit() will dereference the freed
memory block at bs->file afterwards, which is not good.
We can now decide whether to add a "bs->file = NULL;" after each of the
offending bdrv_unref_child() invocations, or just drop them altogether.
The latter is simpler, so let's do that.

Keep the coroutine on the same AioContext. Without this change,
there would be a race between yielding the coroutine and reentering it.
While the race cannot happen now, because the code only runs from a single
AioContext, this will change with multiqueue support in the block layer.
While doing the change, replace custom bottom half with aio_co_schedule.

This enables byte granularity requests for blkdebug, and at the same
time gets us rid of another user of the BDS-level AIO emulation.
Note that unless align=512 is specified, this can behave subtly
different from the old behaviour because bdrv_co_preadv/pwritev don't
have to perform alignment adjustments any more.

In practice the entry argument is always known at creation time, and
it is confusing that sometimes qemu_coroutine_enter is used with a
non-NULL argument to re-enter a coroutine (this happens in
block/sheepdog.c and tests/test-coroutine.c). So pass the opaque value
at creation time, for consistency with e.g. aio_bh_new.

We want to eventually stick request_alignment alongside other
BlockLimits, but first, we must ensure it is populated at the
same time as all other limits, rather than being a special case
that is set only when a block is first opened.
Note that when the user does not provide "align", then we were
defaulting to bs->request_alignment - but at this stage in the
initialization, that was always 512. We were also rejecting an
explicit "align":0 from the user; this patch now allows that,
as an explicit request for the default alignment (which may not
always be 512 in the future).
qemu-iotests 77 is particularly sensitive to the fact that we
can specify an artificial alignment override in blkdebug, and
that override must continue to work even when limits are
refreshed on an already open device.

Commit 57cb38b included qapi/error.h into qemu/osdep.h to get the
Error typedef. Since then, we've moved to include qemu/osdep.h
everywhere. Its file comment explains: "To avoid getting into
possible circular include dependencies, this file should not include
any other QEMU headers, with the exceptions of config-host.h,
compiler.h, os-posix.h and os-win32.h, all of which are doing a
similar job to this file and are under similar constraints."
qapi/error.h doesn't do a similar job, and it doesn't adhere to
similar constraints: it includes qapi-types.h. That's in excess of
100KiB of crap most .c files don't actually need.
Add the typedef to qemu/typedefs.h, and include that instead of
qapi/error.h. Include qapi/error.h in .c files that need it and don't
get it now. Include qapi-types.h in qom/object.h for uint16List.
Update scripts/clean-includes accordingly. Update it further to match
reality: replace config.h by config-target.h, add sysemu/os-posix.h,
sysemu/os-win32.h. Update the list of includes in the qemu/osdep.h
comment quoted above similarly.
This reduces the number of objects depending on qapi/error.h from "all
of them" to less than a third. Unfortunately, the number depending on
qapi-types.h shrinks only a little. More work is needed for that one.

In order to decide whether a blkdebug: filename can be produced or a
json: one is necessary, blkdebug checked whether bs->options had more
options than just "config", "x-image" or "image" (the latter including
nested options). That doesn't work well when generic block layer options
are present.
This patch passes an option QDict to the driver that contains only
driver-specific options, i.e. the options for the general block layer as
well as child nodes are already filtered out. Works much better this
way.

Now that we guarantee the user doesn't have any enum values
beginning with a single underscore, we can use that for our
own purposes. Renaming ENUM_MAX to ENUM__MAX makes it obvious
that the sentinel is generated.

No need to keep two separate enums, where editing one is likely

When the argument is non-zero, qemu_opts_foreach() stops on callback
returning non-zero, and returns that value.
When the argument is zero, it doesn't stop, and returns the bit-wise
inclusive or of all the return values. Funky :)
The callers that pass zero could just as well pass one, because their
callbacks can't return anything but zero:
* qemu_add_globals()'s callback qdev_add_one_global()
* qemu_config_write()'s callback config_write_opts()
* main()'s callbacks default_driver_check(), drive_enable_snapshot(),
vnc_init_func()
Drop the parameter, and always stop.

Background:
The blkdebug scripts are currently engineered so that when a debug
event occurs, a prefilter browses a master list of parsed rules for a
certain event and adds them to an "active list" of rules to be used for
the forthcoming action, provided the events and state numbers match.
Then, once the request is received, the last active rule is used to
inject an error if certain parameters match.
This active list is cleared every time the prefilter injects a new
rule for the first time during a debug event.
The "once" rule currently causes the error injection, if it is
triggered, to only clear the active list. This is insufficient for
preventing future injections of the same rule.
Remedy:
This patch /deletes/ the rule from the list that the prefilter
browses, so it is gone for good. In V2, we remove only the rule of
interest from the active list instead of allowing the "once" rule to
clear the entire list of active rules.
Impact:
This affects iotests 026. Several ENOSPC tests that used "once" can
be seen to have output that shows multiple failure messages. After
this patch, the error messages tend to be smaller and less severe, but
the injection can still be seen to be working. I have patched the
expected output to expect the smaller error messages.

Instead of actually recreating the options from scratch, just reuse the
options given for creating the BDS, which are the configuration file
name and additional options. In case there are no additional options we
can thus create a plain filename.
This obviously results in a different output for qemu-iotest 099 which
exactly tests this filename generation. Fix it up as well.

bdrv_make_empty() is currently only called if the current image
represents an external snapshot that has been committed to its base
image; it is therefore unlikely to have internal snapshots. In this
case, bdrv_make_empty() can be greatly sped up by emptying the L1 and
refcount table (while having the dirty flag set, which only works for
compat=1.1) and creating a trivial refcount structure.
If there are snapshots or for compat=0.10, fall back to the simple
implementation (discard all clusters).

Drop the assumption that we're using the main AioContext. Convert
qemu_bh_new() to aio_bh_new() so we use the BlockDriverState's
AioContext.
The .bdrv_detach_aio_context() and .bdrv_attach_aio_context() interfaces
are not needed since no fd handlers, timers, or BHs stay registered when
requests have been drained.

Make bdrv_open() take a pointer to a BDS pointer, similarly to
bdrv_file_open(). If a pointer to a NULL pointer is given, bdrv_open()
will create a new BDS with an empty name; if the BDS pointer is not
NULL, that existing BDS will be reused (in the same way as bdrv_open()
already did).

error_is_set(&var) is the same as var != NULL, but it takes
whole-program analysis to figure that out. Unnecessarily hard for
optimizers, static checkers, and human readers. Dumb it down to
obvious.
Gets rid of several dozen Coverity false positives.
Note that the obvious form is already used in many places.

If the filename is not prefixed by "blkdebug:" in
blkdebug_parse_filename(), the blkdebug driver was not selected through
that protocol prefix, but by an explicit command line option
(file.driver=blkdebug or something similar). Contrary to the current
reaction, this is not a problem at all; we just need to store the
filename (in the x-image option) and can go on; the user just has to
manually specify the config option.

This allows more systematic AIO testing. The patch adds three new
operations to blkdebug:
* Setting a "breakpoint" on a blkdebug event. The next request that
triggers this breakpoint is suspended and is tagged with a name.
The breakpoint is removed after a request has triggered it.
* A suspended request (identified by it's tag) can be resumed
* It's possible to check whether a suspended request with a given
tag exists. This can be used for waiting for an event.
Ideally, we would instead tag requests right when they are created and
set breakpoints for individual requests. However, at this point the
block layer doesn't allow this easily, and breakpoints that trigger for
any request already allow a lot of useful testing.

The cleanup work to remove a rule depends on the type of the rule. It's
easy for the existing rules as there is no data that must be cleaned up
and is specific to a type yet, but the next patch will change this.

Now that AIOPool no longer keeps a freelist, it isn't really a "pool"
anymore. Rename it to AIOCBInfo and make it const since it no longer
needs to be modified.

Currently it is impossible to write a blkdebug script that ping-pongs
between two states, because the second set-state rule will use the
state that is set in the first. If you have
[set-state]
event = "..."
state = "1"
new_state = "2"
[set-state]
event = "..."
state = "2"
new_state = "1"
for example the state will remain locked at 1. This can be fixed
by first processing all rules, and then setting the state.

This prepares for the next patch, where some active rules may actually
not trigger depending on input to readv/writev. Store the active rules
in a SIMPLEQ (so that it can be emptied easily with QSIMPLEQ_INIT), and
fetch the errno/once/immediately arguments from there.

This is required when using blkdebug with raw format. Unlike qcow2/QED,
raw asks blkdebug for the length of the file, it doesn't get it from
a header.

Most of the codebase as been converted to use glib memory allocation
functions. There are still a few instances of malloc/calloc in the
block layer and qemu-io. Replace them, especially since they do not
check the strdup/malloc/calloc return value.

Block drivers now only need to provide either of .bdrv_co_flush,
.bdrv_aio_flush() or for legacy drivers .bdrv_flush(). Remove
the redundant .bdrv_flush() implementations.

The signedness of enum types depend on the compiler implementation.
Therefore the check for negative values may or may not be meaningful.
Fix by explicitly casting to a signed integer.
Since the values are also checked earlier against event_names
table, this is an internal error. Change the 'if' to 'assert'.
This also avoids a warning with GCC flag -Wtype-limits.

state = 0 in rules means that the rule is valid for any state. Therefore it's
impossible to have a rule that works only in the initial state. This changes
the initial state from 0 to 1 to make this possible.

The list head was initialized to point to the wrong list, so all actions ended
up being handled as inject-error even if they were set-state in fact.

Format drivers shouldn't need to bother with things like file names, but rather
just get an open BlockDriverState for the underlying protocol. This patch
introduces this behaviour for bdrv_open implementation. For protocols which
need to access the filename to open their file/device/connection/... a new
callback bdrv_file_open is introduced which doesn't get an underlying file
opened.
For now, also some of the more obscure formats use bdrv_file_open because they
open() the file themselves instead of using the block.c functions. They need to
be fixed in later patches.

Block drivers can trigger a blkdebug event whenever they reach a place where it
could be useful to inject an error for testing/debugging purposes.
Rules are read from a blkdebug config file and describe which action is taken
when an event is triggered. For now this is only injecting an error (with a few
options) or changing the state (which is an integer). Rules can be declared to
be active only in a specific state; this way later rules can distiguish on
which path we came to trigger their event.

Add a mechanism to inject errors instead of passing requests on. With no
further patches applied, you can use it by setting inject_errno in gdb.

This isn't doing anything interesting. It creates the blkdebug block driver as
a protocol which just passes everything through to raw.
