This is intended to be a semantic revert of commit 9b09503752
("migration: run setup callbacks out of big lock"). There have been so
many changes since that commit (e.g. a new setup callback
dirty_bitmap_save_setup() that also needs to be adapted now), it's
easier to do the revert manually.
For snapshots, the bdrv_writev_vmstate() function is used during setup
(in QIOChannelBlock backing the QEMUFile), but not holding the BQL
while calling it could lead to an assertion failure. To understand
how, first note the following:
1. Generated coroutine wrappers for block layer functions spawn the
coroutine and use AIO_WAIT_WHILE()/aio_poll() to wait for it.
2. If the host OS switches threads at an inconvenient time, it can
happen that a bottom half scheduled for the main thread's AioContext
is executed as part of a vCPU thread's aio_poll().
An example leading to the assertion failure is as follows:
main thread:
1. A snapshot-save QMP command gets issued.
2. snapshot_save_job_bh() is scheduled.
vCPU thread:
3. aio_poll() for the main thread's AioContext is called (e.g. when
the guest writes to a pflash device, as part of blk_pwrite which is a
generated coroutine wrapper).
4. snapshot_save_job_bh() is executed as part of aio_poll().
3. qemu_savevm_state() is called.
4. qemu_mutex_unlock_iothread() is called. Now
qemu_get_current_aio_context() returns 0x0.
5. bdrv_writev_vmstate() is executed during the usual savevm setup
via qemu_fflush(). But this function is a generated coroutine wrapper,
so it uses AIO_WAIT_WHILE. There, the assertion
assert(qemu_get_current_aio_context() == qemu_get_aio_context());
will fail.
To fix it, ensure that the BQL is held during setup. While it would
only be needed for snapshots, adapting migration too avoids additional
logic for conditional locking/unlocking in the setup callbacks.
Writing the header could (in theory) also trigger qemu_fflush() and
thus bdrv_writev_vmstate(), so the locked section also covers the
qemu_savevm_state_header() call, even for migration for consistency.
The section around multifd_send_sync_main() needs to be unlocked to
avoid a deadlock. In particular, the multifd_save_setup() function calls
socket_send_channel_create() using multifd_new_send_channel_async() as a
callback and then waits for the callback to signal via the
channels_ready semaphore. The connection happens via
qio_task_run_in_thread(), but the callback is only executed via
qio_task_thread_result() which is scheduled for the main event loop.
Without unlocking the section, the main thread would never get to
process the task result and the callback meaning there would be no
signal via the channels_ready semaphore.
The comment in ram_init_bitmaps() was introduced by 4987783400
("migration: fix incorrect memory_global_dirty_log_start outside BQL")
and is removed, because it referred to the qemu_mutex_lock_iothread()
call.

Bitmap's source persistence is transported over the migration stream and
the destination mirrors it. In some cases the destination might want to
persist bitmaps which are not persistent on the source (e.g. the result
of merging bitmaps from a number of layers on the source when migrating
into a squashed image) but currently it would need to create another set
of persistent bitmaps and merge them.
This patch adds a 'transform' property to the alias map which allows
overriding the persistence of migrated bitmaps both on the source and

Important thing for bitmap migration is to select destination block
node to obtain the migrated bitmap.
Prepatch, on source we use bdrv_get_device_or_node_name() to identify
the node, and on target we do bdrv_lookup_bs.
bdrv_get_device_or_node_name() returns blk name only for direct
children of blk. So, bitmaps of direct children of blks are migrated by
blk name and others - by node name.
Old libvirt is unprepared to bitmap migration by node-name,
node-names are mostly auto-generated. So actually only migration by blk
name works for it.
Newer libvirt will use new interface (which will be added soon) to
specify node-mapping for bitmaps migration explicitly. Still, let's
improve the current behavior a bit.
Now, consider classic libvirt migrations assisted by mirror block job:
mirror block job inserts filter, so our source is not a direct child of
blk, and bitmaps are migrated by node-names. And this just doesn't work
with auto-generated node names.
Let's fix it by using blk-name even if some implicit filters are
inserted.
Note2: we, of course, can't skip filters and use blk name to migrate
bitmaps in filtered node by blk name for this blk if these filters have
named bitmaps which should be migrated.

sysemu/sysemu.h is a rather unfocused dumping ground for stuff related
to the system-emulator. Evidence:
* It's included widely: in my "build everything" tree, changing
sysemu/sysemu.h still triggers a recompile of some 1100 out of 6600
objects (not counting tests and objects that don't depend on
qemu/osdep.h, down from 5400 due to the previous two commits).
* It pulls in more than a dozen additional headers.
Split stuff related to run state management into its own header
sysemu/runstate.h.
Touching sysemu/sysemu.h now recompiles some 850 objects. qemu/uuid.h
also drops from 1100 to 850, and qapi/qapi-types-run-state.h from 4400
to 4200. Touching new sysemu/runstate.h recompiles some 500 objects.
Since I'm touching MAINTAINERS to add sysemu/runstate.h anyway, also
add qemu/main-loop.h.

Shift from looking at every root BDS to *every* BDS. This will migrate
bitmaps that are attached to blockdev created nodes instead of just ones
attached to emulated storage devices.
Note that this will not migrate anonymous or internal-use bitmaps, as
those are defined as having no name.
This will also fix the Coverity issues Peter Maydell has been asking
about for the past several releases, as well as fixing a real bug.

Instead of checking against busy, inconsistent, or read only directly,
use a check function with permissions bits that let us streamline the
checks without reproducing them in many places.
Included in this patch are permissions changes that simply add the
inconsistent check to existing permissions call spots, without
addressing existing bugs.
In general, this means that busy+readonly checks become BDRV_BITMAP_DEFAULT,
which checks against all three conditions. busy-only checks become
BDRV_BITMAP_ALLOW_RO.
Notably, remove allows inconsistent bitmaps, so it doesn't follow the pattern.

bdrv_dirty_bitmap_busy to indicate semantically what we are describing,

"Frozen" was a good description a long time ago, but it isn't adequate now.
Rename the frozen predicate to has_successor to make the semantics of the
predicate more clear to outside callers.
In the process, remove some calls to frozen() that no longer semantically
make sense. For bdrv_enable_dirty_bitmap_locked and
bdrv_disable_dirty_bitmap_locked, it doesn't make sense to prohibit QEMU
internals from performing this action when we only wished to prohibit QMP
users from issuing these commands. All of the QMP API commands for bitmap
manipulation already check against user_locked() to prohibit these actions.
Several other assertions really want to check that the bitmap isn't in-use
by another operation -- use the bitmap_user_locked function for this instead,
which presently also checks for has_successor. This leaves some redundant
checks of has_successor through different helpers that are addressed in
forthcoming patches.

Most list head structs need not be given a name. In most cases the
name is given just in case one is going to use QTAILQ_LAST, QTAILQ_PREV
or reverse iteration, but this does not apply to lists of other kinds,
and even for QTAILQ in practice this is only rarely needed. In addition,
we will soon reimplement those macros completely so that they do not
need a name for the head struct. So clean up everything, not giving a
name except in the rare case where it is necessary.

This patch aims to bring the following behavior:
1. We don't load bitmaps, when started in inactive mode. It's the case
of incoming migration. In this case we wait for bitmaps migration
through migration channel (if 'dirty-bitmaps' capability is enabled) or
for invalidation (to load bitmaps from the image).
2. We don't remove persistent bitmaps on inactivation. Instead, we only
remove bitmaps after storing. This is the only way to restore bitmaps,
if we decided to resume source after [failed] migration with
'dirty-bitmaps' capability enabled (which means, that bitmaps were not
stored).
3. We load bitmaps on open and any invalidation, it's ok for all cases:
- normal open
- migration target invalidation with dirty-bitmaps capability
(bitmaps are migrating through migration channel, the are not
stored, so they should have IN_USE flag set and will be skipped
when loading. However, it would fail if bitmaps are read-only[1])
- migration target invalidation without dirty-bitmaps capability
(normal load of the bitmaps, if migrated with shared storage)
- source invalidation with dirty-bitmaps capability
(skip because IN_USE)
- source invalidation without dirty-bitmaps capability
(bitmaps were dropped, reload them)
[1]: to accurately handle this, migration of read-only bitmaps is
explicitly forbidden in this patch.
New mechanism for not storing bitmaps when migrate with dirty-bitmaps
capability is introduced: migration filed in BdrvDirtyBitmap.

Instead of both frozen and qmp_locked checks, wrap it into one check.
frozen implies the bitmap is split in two (for backup), and shouldn't
be modified. qmp_locked implies it's being used by another operation,
like being exported over NBD. In both cases it means we shouldn't allow
the user to modify it in any meaningful way.
Replace any usages where we check both frozen and qmp_locked with the
new check.

Bitmap lock/unlock were added to bdrv_enable_dirty_bitmap in
8b1402ce80, but some places were not updated correspondingly, which
leads to trying to take this lock twice, which is dead-lock. Fix this.
Actually, iotest 199 (about dirty bitmap postcopy migration) is broken
now, and this fixes it.
