	(handle_rhs_call): Use it to mark the return slot escaped if
	it is addressable and NRV was applied.

	* g++.dg/torture/pr40389.C: New testcase.

	(output_compilation_unit_header): Emit correct version for dwarf3.

	(output_pubnames): Update for DWARF_VERSION removal.

	(gen_subprogram_die): Emit DW_OP_call_frame_cfa if emitting dwarf3.
