The pseries platform code unconditionally overrides
memory_block_size_bytes regardless of the running platform.
Create a ppc_md hook that so each platform can choose to
do what it wants.

Currently, the code in setup-common.c for powerpc assumes that all
clock rates are same in a smp system. This value is cached in the
variable named ppc_proc_freq and is the value that is reported in
/proc/cpuinfo.
However on the PowerNV platform, the clock rate is same only across
the threads of the same core. Hence the value that is reported in
/proc/cpuinfo is incorrect on PowerNV platforms. We need a better way
to query and report the correct value of the processor clock in
/proc/cpuinfo.
The patch achieves this by creating a machdep_call named
get_proc_freq() which is expected to returns the frequency in Hz. The
code in show_cpuinfo() can invoke this method to display the correct
clock rate on platforms that have implemented this method. On the
other powerpc platforms it can use the value cached in ppc_proc_freq.

The problem was initially reported by Wendy who tried pass through
IPR adapter, which was connected to PHB root port directly, to KVM
based guest. When doing that, pci_reset_bridge_secondary_bus() was
called by VFIO driver and linkDown was detected by the root port.
That caused all PEs to be frozen.
The patch fixes the issue by routing the reset for the secondary bus
of root port to underly firmware. For that, one more weak function
pci_reset_secondary_bus() is introduced so that the individual platforms
can override that and do specific reset for bridge's secondary bus.

The memory remove code for powerpc/pseries should call remove_memory()
so that we are holding the hotplug_memory lock during memory remove
operations.
This patch updates the memory node remove handler to call remove_memory()
and adds a ppc_md.remove_memory() entry to handle pseries specific work
that is called from arch_remove_memory().
During memory remove in pseries_remove_memblock() we have to stay with
removing memory one section at a time. This is needed because of how memory
resources are handled. During memory add for pseries (via the probe file in
sysfs) we add memory one section at a time which gives us a memory resource
for each section. Future patches will aim to address this so will not have
to remove memory one section at a time.

Detect and recover from machine check when inside opal on a special
scom load instructions. On specific SCOM read via MMIO we may get a machine
check exception with SRR0 pointing inside opal. To recover from MC
in this scenario, get a recovery instruction address and return to it from
MC.
OPAL will export the machine check recoverable ranges through
device tree node mcheck-recoverable-ranges under ibm,opal:

Each recoverable address range entry is (start address, len,
recovery address), 2 cells each for start and recovery address, 1 cell for
len, totalling 5 cells per entry. During kernel boot time, build up the
recovery table with the list of recovery ranges from device-tree node which
will be used during machine check exception to recover from MMIO SCOM UE.

The existing TCE machine calls (tce_build and tce_free) only support
virtual mode as they call __raw_writeq for TCE invalidation what
fails in real mode.
This introduces tce_build_rm and tce_free_rm real mode versions
which do mostly the same but use "Store Doubleword Caching Inhibited
Indexed" instruction for TCE invalidation.
This new feature is going to be utilized by real mode support of VFIO.

Add the plumbing to implement arch_get_random_long/int(). It didn't seem
worth adding an extra ppc_md hook for int, so we reuse the one for long.
Add an implementation for powernv based on the hwrng found in power7+
systems. We whiten the output of the hwrng, and the result passes all
the dieharder tests.

If a hash bucket gets full, we "evict" a more/less random entry from it.
When we do that we don't invalidate the TLB (hpte_remove) because we assume
the old translation is still technically "valid". This implies that when
we are invalidating or updating pte, even if HPTE entry is not valid
we should do a tlb invalidate. With hugepages, we need to pass the correct
actual page size value for tlb invalidation.
This change update the patch 0608d69246
"powerpc/mm: Always invalidate tlb on hpte invalidate and update" to handle
transparent hugepages correctly.

On pseries machines the detection for max_bus_speed should be done
through an OpenFirmware property. This patch adds a function to perform
this detection and a hook to perform dynamic adding of the function only
for pseries. This is done by overwriting the weak
pcibios_root_bridge_prepare function which is called by
pci_create_root_bus().

We look at both the segment base page size and actual page size and store
the pte-lp-encodings in an array per base page size.
We also update all relevant functions to take actual page size argument
so that we can use the correct PTE LP encoding in HPTE. This should also
get the basic Multiple Page Size per Segment (MPSS) support. This is needed
to enable THP on ppc64.

Currently the __define_initcall() macro takes three arguments, fn, id and
level. The level argument is exactly the same as the id argument but
wrapped in quotes. To overcome this need to specify three arguments to
the __define_initcall macro, where one argument is the stringification of
another, we can just use the stringification macro instead.

This patch convert different functions to take virtual page number
instead of virtual address. Virtual page number is virtual address
shifted right by VPN_SHIFT (12) bits. This enable us to have an
address range of upto 76 bits.

Rework set_dabr to take a DABRX value as well.
Both the pseries and PS3 hypervisors do some checks on the DABRX
values that are passed in the hcall. This patch stops bogus values
from being passed to hypervisor. Also, in the case where we are
clearing the breakpoint, where DABR and DABRX are zero, we modify the
DABRX value to make it valid so that the hcall won't fail.

Some platforms need to perform resource allocation using a custom algorithm
due to HW constraints, or may want to tweak things globally below a host
bridge. For example OPAL support for IODA will need to perform a
resource allocation pass that applies IODA specific segmentation
constraints to MMIO which cannot be done simply using the kernel generic
resource management code.

The hook dma_get_required_mask is supposed to return the mask required
by the platform to operate efficently. The generic version of
dma_get_required_mask in driver/base/platform.c returns a mask based
only on max_pfn. However, this is likely too big for iommu systems
and could be too small for platforms that require a dma offset or have
a secondary window at a high offset.
Override the default, provide a hook in ppc_md used by pseries lpar and
cell, and provide the default answer based on memblock_end_of_DRAM(),
with hooks for get_dma_offset, and provide an implementation for iommu
that looks at the defined table size. Coverting from the end address
to the required bit mask is based on the generic implementation.
The need for this was discovered when the qla2xxx driver switched to
64 bit dma then reverted to 32 bit when dma_get_required_mask said
32 bits was sufficient.

When we start a cpu we use smp_ops->kick_cpu(), which currently
returns void, it should be able to fail. Convert it to return
int, and update all uses.
Convert all the current error cases to return -ENOENT, which is
what would eventually be returned by __cpu_up() currently when
it doesn't detect the cpu as coming up in time.

Wakeup comes from the system reset handler with a potential loss of
the non-hypervisor CPU state. We save the non-volatile state on the
stack and a pointer to it in the PACA, which the system reset handler
uses to restore things

The current code soft-disables, and then goes to NAP mode which
turns interrupts on. That means that if an interrupt occurs, we
will hit the masked interrupt code path which isn't what we want,
as it will return with EE off, which will either get us out of
NAP mode, or fail to enter it (according to spec).
Instead, let's just rely on the fact that it is safe to take
decrementer interrupts on an offline CPU and leave interrupts
enabled. We can also get rid of the special case in asm for
power4_cpu_offline_powersave() and just use power4_idle().

We use a similar technique to ppc32: We set a thread local flag
to indicate that we are about to enter or have entered the stop
state, and have fixup code in the async interrupt entry code that
reacts to this flag to make us return to a different location
(sets NIP to LINK in our case).

Since the decrementer and timekeeping code was moved over to using
the generic clockevents and timekeeping infrastructure, several
variables and functions have been obsolete and effectively unused.
This deletes them.
In particular, wakeup_decrementer() is no longer needed since the
generic code reprograms the decrementer as part of the process of
resuming the timekeeping code, which happens during sysdev resume.
Thus the wakeup_decrementer calls in the suspend_enter methods for
52xx platforms have been removed. The call in the powermac cpu
frequency change code has been replaced by set_dec(1), which will
cause a timer interrupt as soon as interrupts are enabled, and the
generic code will then reprogram the decrementer with the correct
value.
This also simplifies the generic_suspend_en/disable_irqs functions
and makes them static since they are not referenced outside time.c.
The preempt_enable/disable calls are removed because the generic
code has disabled all but the boot cpu at the point where these
functions are called, so we can't be moved to another cpu.

Version 3 of this patch is updated with documentation added to
Documentation/ABI. There are no changes to any of the C code from v2
of the patch.
In order to support kernel DLPAR of CPU resources we need to provide an
interface to add (probe) and remove (release) the resource from the system.
This patch Creates new generic probe and release sysfs files to facilitate
cpu probe/release. The probe/release interface provides for allowing each
arch to supply their own routines for implementing the backend of adding
and removing cpus to/from the system.
This also creates the powerpc specific stubs to handle the arch callouts
from writes to the sysfs files.
The creation and use of these files is regulated by the
CONFIG_ARCH_CPU_PROBE_RELEASE option so that only architectures that need the
capability will have the files created.

Some of the PCI features we have in ppc32 we will need on ppc64
platforms in the future. These include support for:
* ppc_md.pci_exclude_device
* indirect config cycles
* early config cycles
We also simplified the logic in fake_pci_bus() to assume it will always
get a valid pci_controller. Since all current callers seem to pass it
one.

We essentially adopt the 64-bit dma code, with some changes to support
32-bit systems, including HIGHMEM. dma functions on 32-bit are now
invoked via accessor functions which call the correct op for a device based
on archdata dma_ops. If there is no archdata dma_ops, this defaults
to dma_direct_ops.
In addition, the dma_map/unmap_page functions are added to dma_ops
because we can't just fall back on map/unmap_single when HIGHMEM is
enabled. In the case of dma_direct_*, we stop using map/unmap_single
and just use the page version - this saves a lot of ugly
ifdeffing. We leave map/unmap_single in the dma_ops definition,
though, because they are needed by the iommu code, which does not
implement map/unmap_page. Ideally, going forward, we will completely
eliminate map/unmap_single and just have map/unmap_page, if it's
workable for 64-bit.

Followed by a few documentation/comment fixups and a couple of places
where <asm-powepc/...> was being used explicitly. Of the latter only
one was outside the arch code and it is a driver only built for powerpc.
