ce187c3c15 hw/intc/arm_gicv3: Implement functions to identify next pending irq
Implement the GICv3 logic to recalculate the highest priority pending
interrupt for each CPU after some part of the GIC state has changed.
We avoid unnecessary full recalculation where possible.

This patch includes the device class itself, some ID register
value functions which will be needed by both distributor
and redistributor, and some skeleton functions for handling
interrupts coming in and going out, which will be filled in
in a subsequent patch.

3faf2b0cd5 hw/intc/arm_gicv3: Move irq lines into GICv3CPUState structure
Move the GICv3 parent_irq and parent_fiq pointers into the
GICv3CPUState structure rather than giving them their own array.
This will make it easy to assert the IRQ and FIQ lines for a
particular CPU interface without having to know or calculate
the CPU index for the GICv3CPUState we are working on.

4a4b344c7c xics,xics_kvm: Handle CPU unplug correctly
XICS is setup for each CPU during initialization. Provide a routine
to undo the same when CPU is unplugged. While here, move ss->cs management
into xics from xics_kvm since there is nothing KVM specific in it.
Also ensure xics reset doesn't set irq for CPUs that are already unplugged.

This allows reboot of a VM that has undergone CPU hotplug and unplug
to work correctly.

ed1263c363 ioapic: clear remote irr bit for edge-triggered interrupts
This is to better emulate IOAPIC version 0x1X hardware. Linux kernel
leveraged this "feature" to do explicit EOI since EOI register is still
not introduced at that time. This will also fix the issue that level
triggered interrupts failed to work when IR enabled (tested with Linux
kernel version 4.5).

77ac58ddc6 dma: do not depend on kvm_enabled()
Memory barriers are needed also by Xen and, when the ioeventfd
bugs are fixed, by TCG as well.

sysemu/kvm.h is not anymore needed in sysemu/dma.h, move it to
the actual users.

2531088f6c hw/intc/arm_gic: add tracepoints
These are obviously critical to understanding interrupt delivery:
gic_enable_irq
gic_disable_irq
gic_set_irq (inbound irq from device models)
gic_update_set_irq (outbound irq to CPU)
gic_acknowledge_irq

The only one that I think might raise eyebrows is gic_update_bestirq, but I've
(sadly) debugged problems that ended up being caused by unexpected priorities.
Knowing that the GIC has an irq ready, but doesn't deliver to the CPU due to
priority, has also proven important.

73bcb24d93 Replaced get_tick_per_sec() by NANOSECONDS_PER_SECOND
This patch replaces get_ticks_per_sec() calls with the macro
NANOSECONDS_PER_SECOND. Also, as there are no callers, get_ticks_per_sec()
is then removed.  This replacement improves the readability and
understandability of code.

For example,

    timer_mod(fdctrl->result_timer,
	      qemu_clock_get_ns(QEMU_CLOCK_VIRTUAL) + (get_ticks_per_sec() / 50));

NANOSECONDS_PER_SECOND makes it obvious that qemu_clock_get_ns
matches the unit of the expression on the right side of the plus.

da34e65cb4 include/qemu/osdep.h: Don't include qapi/error.h
Commit 57cb38b included qapi/error.h into qemu/osdep.h to get the
Error typedef.  Since then, we've moved to include qemu/osdep.h
everywhere.  Its file comment explains: "To avoid getting into
possible circular include dependencies, this file should not include
any other QEMU headers, with the exceptions of config-host.h,
compiler.h, os-posix.h and os-win32.h, all of which are doing a
similar job to this file and are under similar constraints."
qapi/error.h doesn't do a similar job, and it doesn't adhere to
similar constraints: it includes qapi-types.h.  That's in excess of
100KiB of crap most .c files don't actually need.

Add the typedef to qemu/typedefs.h, and include that instead of
qapi/error.h.  Include qapi/error.h in .c files that need it and don't
get it now.  Include qapi-types.h in qom/object.h for uint16List.

Update scripts/clean-includes accordingly.  Update it further to match
reality: replace config.h by config-target.h, add sysemu/os-posix.h,
sysemu/os-win32.h.  Update the list of includes in the qemu/osdep.h
comment quoted above similarly.

This reduces the number of objects depending on qapi/error.h from "all
of them" to less than a third.  Unfortunately, the number depending on
qapi-types.h shrinks only a little.  More work is needed for that one.

0c69996e22 hw/intc: Add (new) ASPEED VIC device model
Implement a basic ASPEED VIC device model for the AST2400 SoC[1], with
enough functionality to boot an aspeed_defconfig Linux kernel. The model
implements the 'new' (revised) register set: While the hardware exposes
both the new and legacy register sets, accesses to the model's legacy
register set will not be serviced (however the access will be logged).

a55c910e0b hw/intc/arm_gic.c: Implement GICv2 GICC_DIR
The GICv2 introduces a new CPU interface register GICC_DIR, which
allows an OS to split the "priority drop" and "deactivate interrupt"
parts of interrupt completion. Implement this register.
(Note that the register is at offset 0x1000 in the CPU interface,
which means it is on a different 4K page from all the other registers.)

a005b3ef50 xics: report errors with the QEMU Error API
Using the return value to report errors is error prone:
- xics_alloc() returns -1 on error but spapr_vio_busdev_realize() errors
  on 0
- xics_alloc_block() returns the unclear value of ics->offset - 1 on error
  but both rtas_ibm_change_msi() and spapr_phb_realize() error on 0

This patch adds an errp argument to xics_alloc() and xics_alloc_block() to
report errors. The return value of these functions is a valid IRQ number
if errp is NULL. It is undefined otherwise.

The corresponding error traces get promotted to error messages. Note that
the "can't allocate IRQ" error message in spapr_vio_busdev_realize() also
moves to xics_alloc(). Similar error message consolidation isn't really
applicable to xics_alloc_block() because callers have extra context (device
config address, MSI or MSIX).

This fixes the issues mentioned above.

Similar to the previous patch, it's nice to have all functions
in the tree that involve a visitor and a name for conversion to
or from QAPI to consistently stick the 'name' parameter next
to the Visitor parameter.

Done by manually changing include/qom/object.h and qom/object.c,
then running this Coccinelle script and touching up the fallout
(Coccinelle insisted on adding some trailing whitespace).

51e72bc1dd qapi: Swap visit_* arguments for consistent 'name' placement
JSON uses "name":value, but many of our visitor interfaces were
called with visit_type_FOO(v, &value, name, errp).  This can be
a bit confusing to have to mentally swap the parameter order to
match JSON order.  It's particularly bad for visit_start_struct(),
where the 'name' parameter is smack in the middle of the
otherwise-related group of 'obj, kind, size' parameters! It's
time to do a global swap of the parameter ordering, so that the
'name' parameter is always immediately after the Visitor argument.

Additional reason in favor of the swap: the existing include/qjson.h
prefers listing 'name' first in json_prop_*(), and I have plans to
unify that file with the qapi visitors; listing 'name' first in
qapi will minimize churn to the (admittedly few) qjson.h clients.

Later patches will then fix docs, object.h, visitor-impl.h, and
those clients to match.

Done by first patching scripts/qapi*.py by hand to make generated
files do what I want, then by running the following Coccinelle
script to affect the rest of the code base:

I then had to apply some touchups (Coccinelle insisted on TAB
indentation in visitor.h, and botched the signature of
visit_type_enum() by rewriting 'const char *const strings[]' to

movement of parameters is sufficient to provoke compiler errors
if any callers were missed.

84a3a53cf6 omap: Don't use hw_error() in device init() methods
Device init() methods aren't supposed to call hw_error(), they should
report the error and fail cleanly.  Do that.

The errors are all device misconfiguration.  All callers use
qdev_init_nofail(), so this patch merely converts hw_error() crashes
into &error_abort crashes.  Improvement, because now it crashes closer
to where the misconfiguration bug would be, and a few more bad
examples of hw_error() use are gone.

15eafc2e60 kvm: x86: add support for KVM_CAP_SPLIT_IRQCHIP
This patch adds support for split IRQ chip mode. When
KVM_CAP_SPLIT_IRQCHIP is enabled:

    1.) The PIC, PIT, and IOAPIC are implemented in userspace while
    the LAPIC is implemented by KVM.

    2.) The software IOAPIC delivers interrupts to the KVM LAPIC via
    kvm_set_irq. Interrupt delivery is configured via the MSI routing
    table, for which routes are reserved in target-i386/kvm.c then
    configured in hw/intc/ioapic.c

    3.) KVM delivers IOAPIC EOIs via a new exit KVM_EXIT_IOAPIC_EOI,
    which is handled in target-i386/kvm.c and relayed to the software
    IOAPIC via ioapic_eoi_broadcast.

Upon activating an interrupt, set the corresponding priority bit in the
APR/NSAPR registers without touching the currently set bits. In the event
of nested interrupts, the GIC will then have the information it needs to
restore the priority of the pre-empted interrupt once the higher priority
interrupt finishes execution.

Such mixed-uses make source code inconsistent. This patch removes

favors s->num_cpu. The source is more consistent after this small tweak.

24182fbc19 arm_gic_kvm: Disable live migration if not supported
Currently, if the kernel does not have live migration API, the migration
will still be attempted, but vGIC save/restore functions will just not do
anything. This will result in a broken machine state.

This patch fixes the problem by adding migration blocker if kernel API is
not supported.

After CPU hotplug has been converted to BUS-less hot-plug infrastructure,
the only function ICC bus performs is to propagate reset to LAPICs. However
LAPIC could be reset by registering its reset handler after all device are
initialized.
Do so and drop ~30LOC of not needed anymore ICCBus related code.

When ICC bus/bridge is removed, APIC MMIO will be left
unmapped since it was mapped into system's address space
indirectly by ICC bridge.

4b3cfe72d9 intc/gic: Extract some reusable vGIC code
Some functions previously used only by vGICv2 are useful also for vGICv3
implementation. Untie them from GICState and make accessible from within
other modules:
- kvm_arm_gic_set_irq()
- kvm_gic_supports_attr() - moved to common code and renamed to
  kvm_device_check_attr()
- kvm_gic_access() - turned into GIC-independent kvm_device_access().
  Data pointer changed to void * because some GICv3 registers are
  64-bit wide

Some of these changes are not used right now, but they will be helpful for
implementing live migration.

Actually kvm_dist_get() and kvm_dist_put() could also be made reusable, but
they would require two extra parameters (s->dev_fd and s->num_cpu) as well as
lots of typecasts of 's' to DeviceState * and back to GICState *. This makes
the code very ugly so i decided to stop at this point. I tried also an
approach with making a base class for all possible GICs, but it would contain
only three variables (dev_fd, cpu_num and irq_num), and accessing them through
the rest of the code would be again tedious (either ugly casts or qemu-style
separate object pointer). So i disliked it too.

ff8f06ee76 hw/intc: Implement GIC-500 base class
This class is to be used by both software and KVM implementations of GICv3

Currently it is mostly a placeholder, but in future it is supposed to hold
qemu's representation of GICv3 state, which is necessary for migration.

The interface of this class is fully compatible with GICv2 one. This is
done in order to simplify integration with existing code.

8ff41f3995 hw/intc/arm_gic_common: Configure IRQs as NS if doing direct NS kernel boot
If we directly boot a kernel in NonSecure on a system where the GIC
supports the security extensions then we must cause the GIC to
configure its interrupts into group 1 (NonSecure) rather than the
usual group 0, and with their initial priority set to the highest
NonSecure priority rather than the usual highest Secure priority.
Otherwise the guest kernel will be unable to use any interrupts.

Implement this behaviour, controlled by a flag which we set if
appropriate when the ARM bootloader code calls our ARMLinuxBootIf
interface callback.

d5523a1365 hw/intc/arm_gic: Actually set the active bits for active interrupts
Although we were correctly handling interrupts becoming active
and then inactive, we weren't actually exposing this to the guest
by setting the 'active' flag for the interrupt, so reads
of GICD_ICACTIVERn and GICD_ISACTIVERn would generally incorrectly
return zeroes. Correct this oversight.

The running_irq and last_active arrays represent state which
doesn't exist in a real hardware GIC. The only thing we use
them for is updating the running priority when an interrupt
is completed, but in fact we can use the active-priority
registers to do this. The running priority is always the
priority corresponding to the lowest set bit in the active
priority registers, because only one interrupt at any
particular priority can be active at once.

A GICv2 has both GICC_APR<n> and GICC_NSAPR<n> registers, with
the latter holding the active priority bits for Group 1 interrupts
(usually Nonsecure interrupts), and the Nonsecure view of the
GICC_APR<n> is the second half of the GICC_NSAPR<n> registers.
Turn our half-hearted implementation of APR<n> into a proper
implementation of both APR<n> and NSAPR<n>:

 * Add the underlying state for NSAPR<n>
 * Make sure APR<n> aren't visible for pre-GICv2
 * Implement reading of NSAPR<n>
 * Make non-secure reads of APR<n> behave correctly
 * Implement writing to APR<n> and NSAPR<n>

df92cfa60e hw/intc/arm_gic: Running priority is group priority, not full priority
Priority values for the GIC are divided into a "group priority"
and a "subpriority" (with the division being determined by the
binary point register). The running priority is only determined
by the group priority of the active interrupts, not the
subpriority. In particular, this means that there can't be more
than one active interrupt at any particular group priority.

b06c262b45 armv7m_nvic: Implement ICSR without using internal GIC state
Change the implementation of the Interrupt Control and State Register
in the v7M NVIC to not use the running_irq and last_active internal
state fields in the GIC. These fields don't correspond to state in
a real GIC and will be removed soon.
The changes to the ICSR are:
 * the VECTACTIVE field is documented as identical to the IPSR[8:0]
   field, so implement it that way
 * implement RETTOBASE via looking at the active state bits

c0194a00b0 s390x/kvm: make setting of in-kernel irq routes more efficient
When we add new adapter routes we call kvm_irqchip_add_route() for every
virtqueue and in the same step also do the KVM_SET_GSI_ROUTING ioctl.

This is unnecessary costly as the interface allows us to set multiple
routes in one go. Let's first add all routes to the table stored in the
global kvm_state and then do the ioctl to commit the routes to the
in-kernel irqchip.

This saves us several ioctls to the kernel where for each call a list
is reallocated and populated.

a45863bda9 xics_kvm: Don't enable KVM_CAP_IRQ_XICS if already enabled
When supporting CPU hot removal by parking the vCPU fd and reusing
it during hotplug again, there can be cases where we try to reenable
KVM_CAP_IRQ_XICS CAP for the vCPU for which it was already enabled.
Introduce a boolean member in ICPState to track this and don't
reenable the CAP if it was already enabled earlier.

Re-enabling this CAP should ideally work, but currently it results in
kernel trying to create and associate ICP with this vCPU and that
fails since there is already an ICP associated with it. Hence this
patch is needed to work around this problem in the kernel.

This change allows CPU hot removal to work for sPAPR.

28e0204254 spapr: Merge sPAPREnvironment into sPAPRMachineState
The code for -machine pseries maintains a global sPAPREnvironment structure
which keeps track of general state information about the guest platform.
This predates the existence of the MachineState structure, but performs
basically the same function.

Now that we have the generic MachineState, fold sPAPREnvironment into
sPAPRMachineState, the pseries specific subclass of MachineState.

This is mostly a matter of search and replace, although a few places which
relied on the global spapr variable are changed to find the structure via
qdev_get_machine().

The arm_gic_kvm now calls kvm_irqchip_set_qemuirq_gsi to build
the hash table storing qemu_irq/gsi mappings. From that point on

These probably went unnoticed because most guests will either
never touch them, or will write to them in the process of
configuring the GIC before enabling interrupts.

5cd8cadae8 migration: Use normal VMStateDescriptions for Subsections
We create optional sections with this patch.  But we already have
optional subsections.  Instead of having two mechanism that do the
same, we can just generalize it.

For subsections we just change:

- Add a needed function to VMStateDescription
- Remove VMStateSubsection (after removal of the needed function
  it is just a VMStateDescription)
- Adjust the whole tree, moving the needed function to the corresponding
  VMStateDescription

770c58f8d1 arm_gicv2m: Add GICv2m widget to support MSIs
The ARM GICv2m widget is a little device that handles MSI interrupt
writes to a trigger register and ties them to a range of interrupt lines
wires to the GIC.  It has a few status/id registers and the interrupt wires,
and that's about it.

A board instantiates the device by setting the base SPI number and
number SPIs for the frame.  The base-spi parameter is indexed in the SPI
number space only, so base-spi == 0, means IRQ number 32.  When a device
(the PCI host controller) writes to the trigger register, the payload is
the GIC IRQ number, so we have to subtract 32 from that and then index
into our frame of SPIs.

When instantiating a GICv2m device, tell PCI that we have instantiated
something that can deal with MSIs.  We rely on the board actually wiring
up the GICv2m to the PCI host controller.

dadbb58f59 hw/intc/arm_gic: Add grouping support to gic_update()
Add support to gic_update() for determining the current IRQ
and FIQ status when interrupt grouping is supported. This
simply requires that instead of always raising IRQ we
check the group of the highest priority pending interrupt
and the GICC_CTLR.FIQEn bit to see whether we should raise
IRQ or FIQ.

ICCICR/GICC_CTLR is banked in GICv1 implementations with Security
Extensions or in GICv2 in independent from Security Extensions.
This makes it possible to enable forwarding of interrupts from
the CPU interfaces to the connected processors for Group0 and Group1.

We also allow to set additional bits like AckCtl and FIQEn by changing
the type from bool to uint32. Since the field does not only store the
enable bit anymore and since we are touching the vmstate, we use the
opportunity to rename the field to cpu_ctlr.

This register is banked in GICs with Security Extensions. Storing the
non-secure copy of BPR in the abpr, which is an alias to the non-secure
copy for secure access. ABPR itself is only accessible from secure state
if the GIC implements Security Extensions.

679aa175e8 hw/intc/arm_gic: Make ICDDCR/GICD_CTLR banked
ICDDCR/GICD_CTLR is banked if the GIC has the security extensions,
and the S (or only) copy has separate enable bits for Group0 and
Group1 enable if the GIC implements interrupt groups.

EnableGroup0 (Bit [1]) in GICv1 is architecturally IMPDEF. Since this
bit (Enable Non-secure) is present in the integrated GIC of the Cortex-A9
MPCore, we support this bit in our GICv1 implementation too.

Now that the GIC base class has state fields for the GICD_IGROUPRn
registers, make kvm_arm_gic_get() and kvm_arm_gic_put() write and

c27a5ba948 hw/intc/arm_gic: Add Interrupt Group Registers
The Interrupt Group Registers allow the guest to configure interrupts
into one of two groups, where Group0 are higher priority and may
be routed to IRQ or FIQ, and Group1 are lower priority and always
routed to IRQ. (In a GIC with the security extensions Group0 is
Secure interrupts and Group 1 is NonSecure.)
The GICv2 always supports interrupt grouping; the GICv1 does only
if it implements the security extensions.

This patch implements the ability to read and write the registers;
the actual functionality the bits control will be added in a
subsequent patch.

165cdaf857 armv7m_nvic: systick: Reload the RELOAD value and count down only if ENABLE bit is set
Consider the following pseudo code to configure SYSTICK (The
recommended programming sequence from "the definitive guide to the
arm cortex-m3"):
    SYSTICK Reload Value Register = 0xffff
    SYSTICK Current Value Register = 0
    SYSTICK Control and Status Register = 0x7

The pseudo code "SYSTICK Current Value Register = 0" leads to invoking
systick_reload(). As a consequence, the systick.tick member is updated
and the systick timer starts to count down when the ENABLE bit of
SYSTICK Control and Status Register is cleared.

The worst case is that: during the system initialization, the reset
value of the SYSTICK Control and Status Register is 0x00000000.
When the code "SYSTICK Current Value Register = 0" is executed, the
systick.tick member is accumulated with "(s->systick.reload + 1) *
systick_scale(s)". The systick_scale() gets the external_ref_clock
scale because the CLKSOURCE bit of the SYSTICK Control and Status
Register is cleared. This is the incorrect behavior because of the
code "SYSTICK Control and Status Register = 0x7". Actually, we want
the processor clock instead of the external reference clock.

This incorrect behavior defers the generation of the first interrupt.

The patch fixes the above-mentioned issue by setting the systick.tick
member and modifying the systick timer only if the ENABLE bit of
the SYSTICK Control and Status Register is set.

In addition, the Cortex-M3 Devices Generic User Guide mentioned that
"When ENABLE is set to 1, the counter loads the RELOAD value from the
SYST RVR register and then counts down". This patch adheres to the
statement of the user guide.

9cb11fd753 target-i386: clear bsp bit when designating bsp
Since the BSP bit is writable on real hardware, during reset all the CPUs which
were not chosen to be the BSP should have their BSP bit cleared. This fix is
required for KVM to work correctly when it changes the BSP bit.

An additional fix is required for QEMU tcg to allow software to change the BSP
bit.

It uses a new group/attribute that allows such operation:

ae4a2bd759 s390x: Replace unchecked qdev_init() by qdev_init_nofail()
s390_flic_init() is a helper to create and realize either
"s390-flic-kvm" or "s390-flic-qemu".  When qdev_init() fails, it
complains to stderr and succeeds.

Except it can't actually fail, because the "s390-flic-qemu" is a dummy
without a realize method, and "s390-flic-kvm"'s realize can't fail,
even when the kernel device is really unavailable.  Odd.

Replace qdev_init() by qdev_init_nofail() to make "can't fail" locally
obvious, and get rid of the unreachable error reporting.

032ab09290 openpic: fix up loadvm under -M mac99
Issuing loadvm under -M mac99 would fail for two reasons: firstly an incorrect
version number for openpic would cause openpic_load() to abort, and secondly
a cut/paste error when restoring the IVPR and IDR registers caused subsequent
vmstate sections to become misaligned and abort early.

1ce6be24df hmp: Name HMP info handler functions hmp_info_SUBCOMMAND()
Some are called do_info_SUBCOMMAND() (old ones, usually), some
hmp_info_SUBCOMMAND(), some SUBCOMMAND_info(), sometimes SUBCOMMAND
pointlessly differs in spelling.

Normalize to hmp_info_SUBCOMMAND(), where SUBCOMMAND is exactly the
subcommand name with '-' replaced by '_'.

Exceptions:

* sun4m_irq_info(), sun4m_pic_info() renamed to sun4m_hmp_info_irq(),
  sun4m_hmp_info_pic().

* lm32_irq_info(), lm32_pic_info() renamed to lm32_hmp_info_irq(),
  lm32_hmp_info_pic().

927411fa42 apic: do not dereference pointer before it is checked for NULL
Right now you only get to apic_init_reset if you have an APIC
(do_cpu_init is reached only if CPU_INTERRUPT_INIT is set and
that only happens in hw/intc/apic.c).  However, this is wrong
because for example a port 92 or keyboard controller reset is
really an INIT, and that can happen also with no APIC.  So
keep the check and fix the error that Coverity reported.

f41389ae3c KVM_CAP_IRQFD and KVM_CAP_IRQFD_RESAMPLE checks
Compute kvm_irqfds_allowed by checking the KVM_CAP_IRQFD extension.
Remove direct settings in architecture specific files.

Add a new kvm_resamplefds_allowed variable, initialized by
checking the KVM_CAP_IRQFD_RESAMPLE extension. Add a corresponding
kvm_resamplefds_enabled() function.

A special notice for s390 where KVM_CAP_IRQFD was not immediatly
advirtised when irqfd capability was introduced in the kernel.
KVM_CAP_IRQ_ROUTING was advertised instead.

This was fixed in "KVM: s390: announce irqfd capability",
ebc3226202d5956a5963185222982d435378b899 whereas irqfd support
was brought in 84223598778ba08041f4297fda485df83414d57e,
"KVM: s390: irq routing for adapter interrupts".  Both commits
first appear in 3.15 so there should not be any kernel
version impacted by this QEMU modification.

5224c88dd3 apic: fix incorrect handling of ExtINT interrupts wrt processor priority
This fixes another failure with ExtINT, demonstrated by QNX.  The failure
mode is as follows:
- IPI sent to cpu 0 (bit set in APIC irr)
- IPI accepted by cpu 0 (bit cleared in irr, set in isr)
- IPI sent to cpu 0 (bit set in both irr and isr)
- PIC interrupt sent to cpu 0

The PIC interrupt causes CPU_INTERRUPT_HARD to be set, but
apic_irq_pending observes that the highest pending APIC interrupt priority
(the IPI) is the same as the processor priority (since the IPI is still
being handled), so apic_get_interrupt returns a spurious interrupt rather
than the pending PIC interrupt. The result is an endless sequence of
spurious interrupts, since nothing will clear CPU_INTERRUPT_HARD.

Instead, ExtINT interrupts should have ignored the processor priority.
Calling apic_check_pic early in apic_get_interrupt ensures that
apic_deliver_pic_intr is called instead of delivering the spurious
interrupt.  apic_deliver_pic_intr then clears CPU_INTERRUPT_HARD if needed.

8092cb7132 apic: fix loss of IPI due to masked ExtINT
This patch fixes an obscure failure of the QNX kernel on QEMU x86 SMP.
In QNX, all hardware interrupts come via the PIC, and are delivered by
the cpu 0 LAPIC in ExtINT mode, while IPIs are delivered by the LAPIC
in fixed mode.

This bug happens as follows:
- cpu 0 masks a particular PIC interrupt
- IPI sent to cpu 0 (CPU_INTERRUPT_HARD is set)
- before the IPI is accepted, the masked interrupt line is asserted by the
device

Since the interrupt is masked, apic_deliver_pic_intr will clear
CPU_INTERRUPT_HARD. The IPI will still be set in the APIC irr, but since
CPU_INTERRUPT_HARD is not set the cpu will not notice. Depending on the
scenario this can cause a system hang, i.e. if cpu 0 is expected to unmask
the interrupt.

In order to fix this, do a full check of the APIC before an EXTINT
is acknowledged.  This can result in clearing CPU_INTERRUPT_HARD, but
can also result in delivering the lost IPI.

60e68042cf apic: avoid getting out of halted state on masked PIC interrupts
After the next patch, if a masked PIC interrupts causes CPU_INTERRUPT_POLL
to be set, the CPU will spuriously get out of halted state.  While this
is technically valid, we should avoid that.

Make CPU_INTERRUPT_POLL run apic_update_irq in the right thread and then
look at CPU_INTERRUPT_HARD.  If CPU_INTERRUPT_HARD does not get set,
do not report the CPU as having work.

Also move the handling of software-disabled APIC from apic_update_irq
to apic_irq_pending, and always trigger CPU_INTERRUPT_POLL.  This will
be important once we will add a case that resets CPU_INTERRUPT_HARD
from apic_update_irq.  We want to run it even if we go through
CPU_INTERRUPT_POLL, and even if the local APIC is software disabled.

9ac58dc59a PPC: openpic_kvm: Only map first occurence in address space
The in-kernel OpenPIC emulation only supports a single map. However, we
map the OpenPIC at 2 locations: The CPU visible one and the PCI visible
one. For KVM acceleration, we only care about the first one.

To make sure that we only map that first mapping and not the PCI map that
happens dynamically later during bootup, ignore maps that happen when
we are already considering ourselves mapped.

bee763dbfb spapr: Move interrupt allocator to xics
The current allocator returns IRQ numbers from a pool and does not
support IRQs reuse in any form as it did not keep track of what it
previously returned, it only keeps the last returned IRQ. Some use
cases such as PCI hot(un)plug may require IRQ release and reallocation.

This moves an allocator from SPAPR to XICS.

This switches IRQ users to use new API.

This uses LSI/MSI flags to know if interrupt is allocated.

The interrupt release function will be posted as a separate patch.
