Switch the creation of the combiner devices to the new-style
"embedded in state struct" approach, so we can easily refer
to the object elsewhere during realize.

The function exynos4210_combiner_get_gpioin() currently lives in
exynos4210_combiner.c, but it isn't really part of the combiner
device itself -- it is a function that implements the wiring up of
some interrupt sources to multiple combiner inputs. Move it to live
with the other SoC-level code in exynos4210.c, along with a few
macros previously defined in exynos4210.h which are now used only
in exynos4210.c.

Some typedefs and macros are defined after the type check macros.
This makes it difficult to automatically replace their
definitions with OBJECT_DECLARE_TYPE.
Patch generated using:
$ ./scripts/codeconverter/converter.py -i \
--pattern=QOMStructTypedefSplit $(git grep -l '' -- '*.[ch]')
which will split "typdef struct { ... } TypedefName"
declarations.
Followed by:
$ ./scripts/codeconverter/converter.py -i --pattern=MoveSymbols \
$(git grep -l '' -- '*.[ch]')
which will:
- move the typedefs and #defines above the type check macros
- add missing #include "qom/object.h" lines if necessary

In my "build everything" tree, changing hw/qdev-properties.h triggers
a recompile of some 2700 out of 6600 objects (not counting tests and
objects that don't depend on qemu/osdep.h).
Many places including hw/qdev-properties.h (directly or via hw/qdev.h)
actually need only hw/qdev-core.h. Include hw/qdev-core.h there
instead.
hw/qdev.h is actually pointless: all it does is include hw/qdev-core.h
and hw/qdev-properties.h, which in turn includes hw/qdev-core.h.
Replace the remaining uses of hw/qdev.h by hw/qdev-properties.h.
While there, delete a few superfluous inclusions of hw/qdev-core.h.
Touching hw/qdev-properties.h now recompiles some 1200 objects.

In my "build everything" tree, changing hw/hw.h triggers a recompile
of some 2600 out of 6600 objects (not counting tests and objects that
don't depend on qemu/osdep.h).
The previous commits have left only the declaration of hw_error() in
hw/hw.h. This permits dropping most of its inclusions. Touching it
now recompiles less than 200 objects.

hw/hw.h supposedly includes it for convenience. Several other headers
include it just to get VMStateDescription. The previous commit made
that unnecessary.
Include migration/vmstate.h only where it's still needed. Touching it
now recompiles only some 1600 objects.

hw/hw.h supposedly includes it for convenience. Several other headers
include it just to get qemu_irq and.or qemu_irq_handler.
Move the qemu_irq and qemu_irq_handler typedefs from hw/irq.h to
qemu/typedefs.h, and then include hw/irq.h only where it's still
needed. Touching it now recompiles only some 500 objects.

After commit 767adce2d, they are redundant. This way we don't assign them
except when needed. Once there, there were lots of cases where the ".fields"
indentation was wrong:
.fields = (VMStateField []) {
and
.fields = (VMStateField []) {
Change all the combinations to:
.fields = (VMStateField[]){
The biggest problem (apart from aesthetics) was that checkpatch complained
when we copy&pasted the code from one place to another.

The Exynos4210 combiner has IIC_NIRQ inputs and IIC_NGRP outputs;
use the correct constant in the loop initializing our output
sysbus IRQs so that we don't overrun the output_irq[] array.
