This commit was generated mechanically by coccinelle from the following
semantic patch:

The call sites have been audited to ensure the ffs(0) - 1 == -1 case
never occurs (due to input validation, asserts, etc).  Therefore we

Max Reitz 14a58a4e0c qcow2: Respect new_block in alloc_refcount_block()
When choosing a new place for the refcount table, alloc_refcount_block()
tries to infer the number of clusters used so far from its argument
cluster_index (which comes from the idea that if any cluster with an
index greater than cluster_index was in use, the refcount table would
have to be big enough already to describe cluster_index).

However, there is a cluster that may be at or after cluster_index, and
which is not covered by the refcount structures, and that is the new
refcount block new_block. Therefore, it should be taken into account for
the blocks_used calculation.

Also, because new_block already describes (or is intended to describe)
cluster_index, we may not put the new refcount structures there.

Max Reitz 7453c96b78 qcow2: Helper function for refcount modification
Since refcounts do not always have to be a uint16_t, all refcount blocks
and arrays in memory should not have a specific type (thus they become
pointers to void) and for accessing them, two helper functions are used
(a getter and a setter). Those functions are called indirectly through
function pointers in the BDRVQcowState so they may later be exchanged
for different refcount orders.

With the check and repair functions using this function, the refcount
array they are creating will be in big endian byte order; additionally,
using realloc_refcount_array() makes the size of this refcount array
always cluster-aligned. Both combined allow rebuild_refcount_structure()
to drop the bounce buffer which was used to convert parts of the
refcount array to big endian byte order and store them on disk. Instead,
those parts can now be written directly.

Max Reitz 5fee192efd qcow2: Helper for refcount array reallocation
Add a helper function for reallocating a refcount array, independent of
the refcount order. The newly allocated space is zeroed and the function
handles failed reallocations gracefully.

The helper function will always align the buffer size to a cluster
boundary; if storing the refcounts in such an array in big endian byte
order, this makes it possible to write parts of the array directly as
refcount blocks into the image file.

Max Reitz 2aabe7c7a1 qcow2: Use unsigned addend for update_refcount()
update_refcount() and qcow2_update_cluster_refcount() currently take a
signed addend. At least one caller passes a value directly derived from
an absolute refcount that should be reached ("l2_refcount - 1" in
expand_zero_clusters_in_l1()). Therefore, the addend should be unsigned
as well; this will be especially important for 64 bit refcounts.

Because update_refcount() then no longer knows whether the refcount
should be increased or decreased, it now requires an additional flag
which specified exactly that. The same applies to
qcow2_update_cluster_refcount().

Max Reitz 7324c10f96 qcow2: Only return status from qcow2_get_refcount
Refcounts can theoretically be of type uint64_t; in order to be able to
represent the full range, qcow2_get_refcount() cannot use a single
variable to represent both all refcount values and also keep some values
reserved for errors.

One solution would be to add an Error pointer parameter to
qcow2_get_refcount(); however, no caller could (currently) pass that
error message, so it would have to be emitted immediately and be
passed to the next caller by returning -EIO or something similar.
Therefore, an Error parameter does not offer any advantages here.

The solution applied by this patch is simpler to use. Because no caller
would be able to pass the error message, they would have to print it and
free it, whereas with this patch the caller only needs to pass the
returned integer (which is often a no-op from the code perspective,
because that integer will be stored in a variable "ret" which will be
returned by the fail path of many callers).

Max Reitz c6e9d8ae66 qcow2: Do not return new value after refcount update
qcow2_update_cluster_refcount() does not have any quick access to the
new refcount value, it has to call qcow2_get_refcount(). Some callers do
not need that new value at all, others call qcow2_get_refcount()
themselves anyway (albeit in a different code path, which can however be
easily changed), therefore there is no advantage in making
qcow2_update_cluster_refcount() return the new value. Drop it.

Max Reitz 44751917db block/qcow2: Make get_refcount() global
Reading the refcount of a cluster is an operation which can be useful in
all of the qcow2 code, so make that function globally available.

While touching this function, amend the comment describing the "addend"
parameter: It is (no longer, if it ever was) necessary to have it set to
-1 or 1; any value is fine.

Max Reitz c7c0681bc8 qcow2: Rebuild refcount structure during check
The previous commit introduced the "rebuild" variable to qcow2's
implementation of the image consistency check. Now make use of this by
adding a function which creates a completely new refcount structure
based solely on the in-memory information gathered before.

The old refcount structure will be leaked, however. This leak will be
dealt with in a follow-up commit.

Max Reitz f307b2558f qcow2: Do not perform potentially damaging repairs
If a referenced cluster has a refcount of 0, increasing its refcount may
result in clusters being allocated for the refcount structures. This may
overwrite the referenced cluster, therefore we cannot simply increase
the refcount then.

In such cases, we can either try to replicate all the refcount
operations solely for the check operation, basing the allocations on the
in-memory refcount table; or we can simply rebuild the whole refcount
structure based on the in-memory refcount table. Since the latter will
be much easier, do that.

To prepare for this, introduce a "rebuild" boolean which should be set
to true whenever a fix is rather dangerous or too complicated using the
current refcount structures. Another example for this is refcount blocks
being referenced more than once.

As of a future patch, inc_refcounts() will have to throw errors which
are generally signaled by returning -errno. Therefore, let it return an
integer which is either 0 for success or -errno and handle the -errno

Instead of printing out an error message, incrementing check_errors and
returning a fixed -errno, just do cleanups and return -ret, with ret set
by the code which threw the exception (jumped to the fail label).

Offsets taken from the L1, L2 and refcount tables are generally assumed
to be correctly aligned. However, this cannot be guaranteed if the image
has been written to by something different than qemu, thus check all
offsets taken from these tables for correct cluster alignment.

Max Reitz 9bf040b962 qapi/block: Add "fatal" to BLOCK_IMAGE_CORRUPTED
Not every BLOCK_IMAGE_CORRUPTED event must be fatal; for example, when
reading from an image, they should generally not be. Nonetheless, even
an image only read from may of course be corrupted and this can be
detected during normal operation. In this case, a non-fatal event should
be emitted, but the image should not be marked corrupt (in accordance to
"fatal" set to false).

Markus Armbruster 5839e53bbc block: Use g_new() & friends where that makes obvious sense
g_new(T, n) is neater than g_malloc(sizeof(T) * n).  It's also safer,
for two reasons.  One, it catches multiplication overflowing size_t.
Two, it returns T * rather than void *, which lets the compiler catch
more type errors.

Patch created with Coccinelle, with two manual changes on top:

* Add const to bdrv_iterate_format() to keep the types straight

* Convert the allocation in bdrv_drop_intermediate(), which Coccinelle

Stefan Hajnoczi 39ba3bf69c qcow2: fix new_blocks double-free in alloc_refcount_block()
Commit de82815db1 ("qcow2: Handle failure
for potentially large allocations") introduced a double-free of
new_blocks in the alloc_refcount_block() error path.

The qemu-iotests qcow2 026 test case was failing because qemu-io
segfaulted.

Make sure new_blocks is NULL after we free it the first time.

Max Reitz 8fcffa9853 qcow2: Return useful error code in refcount_init()
If bdrv_pread() returns an error, it is very unlikely that it was
ENOMEM. In this case, the return value should be passed along; as
bdrv_pread() will always either return the number of bytes read or a
negative value (the error code), the condition for checking whether
bdrv_pread() failed can be simplified (and clarified) as well.

Kevin Wolf de82815db1 qcow2: Handle failure for potentially large allocations
Some code in the block layer makes potentially huge allocations. Failure
is not completely unexpected there, so avoid aborting qemu and handle
out-of-memory situations gracefully.

This patch addresses the allocations in the qcow2 block driver.

Max Reitz 65f33bc002 qcow2: Fix alloc_clusters_noref() overflow detection
If the very first allocation has a length of 0, the free_cluster_index
is still 0 after the for loop, which means that subtracting one from it
will underflow and signal an invalid range of clusters by returning
-EFBIG. However, there is no such range, as its length is 0.

Fix this by preventing underflows on free_cluster_index during the
check.

Max Reitz 91f827dcff qcow2: Avoid overflow in alloc_clusters_noref()
alloc_clusters_noref() stores the cluster index in a uint64_t. However,
offsets are often represented as int64_t (as for example the return
value of alloc_clusters_noref() itself demonstrates). Therefore, we
should make sure all offsets in the allocated range of clusters are
representable using int64_t without overflows.

Kevin Wolf b106ad9185 qcow2: Don't rely on free_cluster_index in alloc_refcount_block() (CVE-2014-0147)
free_cluster_index is only correct if update_refcount() was called from
an allocation function, and even there it's brittle because it's used to
protect unfinished allocations which still have a refcount of 0 - if it
moves in the wrong place, the unfinished allocation can be corrupted.

So not using it any more seems to be a good idea. Instead, use the
first requested cluster to do the calculations. Return -EAGAIN if
unfinished allocations could become invalid and let the caller restart
its search for some free clusters.

The context of creating a snapsnot is one situation where
update_refcount() is called outside of a cluster allocation. For this
case, the change fixes a buffer overflow if a cluster is referenced in
an L2 table that cannot be represented by an existing refcount block.
(new_table[refcount_table_index] was out of bounds)

[Bump the qemu-iotests 026 refblock_alloc.write leak count from 10 to
11.

Max Reitz a134d90f50 qcow2: Fix fail path in realloc_refcount_block()
If qcow2_alloc_clusters() fails, new_offset and ret will both be
negative after the fail label, thus passing the first if condition and
subsequently resulting in a call of qcow2_free_clusters() with an
invalid (negative) offset parameter. Fix this by introducing a new label
"fail_free_cluster" which is only invoked if new_offset is indeed
pointing to a newly allocated cluster that should be cleaned up by
freeing it.

While we're at it, clean up the whole fail path. qcow2_cache_put()
should (and actually can) never fail, hence the return value can safely
be ignored (aside from asserting that it indeed did not fail).

Furthermore, there is no reason to give QCOW2_DISCARD_ALWAYS to
qcow2_free_clusters(), a mere QCOW2_DISCARD_OTHER will suffice.

Ultimately, rename the "fail" label to "done", as it is invoked both on
failure and success.

Hu Tao 33304ec9fa qcow2: fix offset overflow in qcow2_alloc_clusters_at()
When cluster size is big enough it can lead to an offset overflow
in qcow2_alloc_clusters_at(). This patch fixes it.

The allocation is stopped each time at L2 table boundary
(see handle_alloc()), so the possible maximum bytes could be

  2^(cluster_bits - 3 + cluster_bits)

cluster_bits - 3 is used to compute the number of entry by L2
and the additional cluster_bits is to take into account each
clusters referenced by the L2 entries.

so int is safe for cluster_bits<=17, unsafe otherwise.

Max Reitz 998b959c1e qcow2: Use pread for inactive L1 in overlap check
Currently, qcow2_check_metadata_overlap uses bdrv_read to read inactive
L1 tables from disk. The number of sectors to read is calculated through
a truncating integer division, therefore, if the L1 table size is not a
multiple of the sector size, the final entries will not be read and
their entries in memory remain undefined (from the g_malloc).
Using bdrv_pread fixes this.

Max Reitz a40f1c2add qcow2: Metadata overlap checks
Two new functions are added; the first one checks a given range in the
image file for overlaps with metadata (main header, L1 tables, L2
tables, refcount table and blocks).

The second one should be used immediately before writing to the image
file as it calls the first function and, upon collision, marks the
image as corrupt and makes the BDS unusable, thereby preventing
further access.

Both functions take a bitmask argument specifying the structures which
should be checked for overlaps, making it possible to also check
metadata writes against colliding with other structures.

This optimises the discard operation for freed clusters by batching
discard requests (both snapshot deletion and bdrv_discard end up
updating the refcounts cluster by cluster).

Note that we don't discard asynchronously, but keep s->lock held. This
is to avoid that a freed cluster is reallocated and written to while the
discard is still in flight.

In error cases, there's no point in writing an updated L1 table, so

This should be based on the virtual disk size, not on the size of the
image.

Interesting observation: With some VM state stored in the image file,
percentages higher than 100% are possible, even though snapshots
themselves are ignored. This is a qcow2 bug to be fixed another day: The
VM state should be discarded in the active L2 tables after completing
the snapshot creation.

The update_cluster_refcount() function increments/decrements a cluster's
refcount and then returns the new refcount value.

There is no need to flush since both update_cluster_refcount() callers
already take care of this:

1. qcow2_alloc_bytes() calls update_cluster_refcount() when compressed
   sectors will be appended to an existing cluster with enough free
   space.  qcow2_alloc_bytes() already flushes so there is no need to do
   so in update_cluster_refcount().

2. qcow2_update_snapshot_refcount() sets a cache dependency on refcounts
   if it needs to update L2 entries.  It also flushes before completing.

Removing this flush significantly speeds up qcow2 snapshot creation:

  $ qemu-img create -f qcow2 test.qcow2 -o size=50G,preallocation=metadata
  $ time qemu-img snapshot -c new test.qcow2

Time drops from more than 3 minutes to under 1 second.

The qemu-img check command can display fragmentation statistics:
 * Total number of clusters in virtual disk
 * Number of allocated clusters
 * Number of fragmented clusters

This patch adds fragmentation statistics support to qcow2.

Compressed and normal clusters count as allocated.  Zero clusters are
not counted as allocated unless their L2 entry has a non-zero offset
(e.g. preallocation).

Only the current L1 table counts towards the statistics - snapshots are
ignored.

Someone forgot something in commit 29c1a730... Documenting the right
return value is not enough, you also need to actually return it in the
code.

This bug sometimes causes error return values even when everything has
succeeded: The new offset of the refcount block is truncated to 32 bits

After it is done with updating refcounts in the cache, update_refcount writes
all changed entries to disk. If a refcount block allocation fails, however,
there was no change yet and therefore first_index = last_index = -1. Don't
treat -1 as a normal sector index (resulting in a 512 byte write!) but return
without updating anything in this case.

Kevin Wolf 66f82ceed6 block: Open the underlying image file in generic code
Format drivers shouldn't need to bother with things like file names, but rather
just get an open BlockDriverState for the underlying protocol. This patch
introduces this behaviour for bdrv_open implementation. For protocols which
need to access the filename to open their file/device/connection/... a new
callback bdrv_file_open is introduced which doesn't get an underlying file
opened.

For now, also some of the more obscure formats use bdrv_file_open because they
open() the file themselves instead of using the block.c functions. They need to
be fixed in later patches.

Kevin Wolf 92dcb59fd4 qcow2: Rewrite alloc_refcount_block/grow_refcount_table
The current implementation of alloc_refcount_block and grow_refcount_table has
fundamental problems regarding error handling. There are some places where an
I/O error means that the image is going to be corrupted. I have found that the
only way to fix this is to completely rewrite the thing.

In detail, the problem is that the refcount blocks itself are allocated using
alloc_refcount_noref (to avoid endless recursion when updating the refcount of
the new refcount block, which migh access just the same refcount block but its
allocation is not yet completed...). Only at the end of the refcount allocation
the refcount of the refcount block is increased. If an error happens in
between, the refcount block is in use, but has a refcount of zero and will
likely be overwritten later.

The new approach is explained in comments in the code. The trick is basically
to let new refcount blocks describe their own refcount, so their refcount will
be automatically changed when they are hooked up in the refcount table.

When the refcount table grows, it doesn't only grow by one entry but reserves
some space for future refcount blocks. The algorithm to calculate the number of
entries stays the same with the fixes, so factor it out before replacing the
rest.

As Juan suggested take the opportunity to simplify the code a bit.

Kevin Wolf 80ee15a6b2 qcow2: Increase maximum cluster size to 2 MB
This patch increases the maximum qcow2 cluster size to 2 MB. Starting with 128k
clusters, L2 tables span 2 GB or more of virtual disk space, causing 32 bit
truncation and wraparound of signed integers. Therefore some variables need to
use a larger data type.

While being at reviewing data types, change some integers that are used for
array indices to unsigned. In some places they were checked against some upper
limit but not for negative values. This could avoid potential segfaults with
corrupted qcow2 images.

The really time consuming part of snapshotting is to adjust the reference count
of all clusters. Currently after each adjusted cluster the refcount block is
written to disk.

Don't write each single byte immediately to disk but cache all writes to the

The qcow2 source is now split into several more manageable files. During the
conversion quite some functions that were static before needed to be changed to
be global to make the source compile again.

We were lucky enough not to get name conflicts with these additional global
names, but they are not nice. This patch adds a qcow2_ prefix to all of the
global functions in qcow2.
