X86: Ignore REX_B bit for 32-bit XOP instructions

While decoding 32-bit XOP instructions, 64 bit registers names are printed.
This patch fixes this by ignoring REX_B bit in 32-bit mode.

X86: Remove the .s suffix from EVEX vpextrw

The .s suffix indicates that the instruction is encoded by swapping
2 register operands. Since vpextrw takes an XMM register and an
integer register, the .s suffix should be ignored for EVEX vpextrw.

X86: Properly handle bad FPU opcode

Since Bad_Opcode and FGRPd9_2 were the same in i386-dis.c, all
Bad_Opcode entries in float_reg were displaced as FGRPd9_2. This
patch adds an entry for Bad_Opcode in fgrps to avoid treating it
as FGRPd9_2.

In 32-bit, the REX_B bit in the 3-byte VEX prefix is ignored and the
the highest bit in VEX.vvvv is either 1 or ignored. In 64-bit, we
need to check invalid mask registers.

AMD64 spec and Intel64 spec differ in indirect branches in 64-bit mode.
AMD64 supports indirect branches with 16-bit address via the data size
prefix while the data size prefix is ignored by Intel64.

Support AMD64/Intel ISAs in assembler/disassembler
...
AMD64 spec and Intel64 spec differ in direct unconditional branches in
64-bit mode. AMD64 supports direct unconditional branches with 16-bit
offset via the data size prefix, which truncates RIP to 16 bits, while
the data size prefix is ignored by Intel64.
This patch adds -mamd64/-mintel64 option to x86-64 assembler and
-Mamd64/-Mintel64 option to x86-64 disassembler. The most permissive
ISA, which is AMD64, is the default.
GDB can add an option, similar to
(gdb) help set disassembly-flavor
Set the disassembly flavor.
The valid values are "att" and "intel", and the default value is "att".
to select which ISA to disassemble.

x86: disambiguate disassembly of certain AVX512 insns

Certain conversion operations as well as vfpclassp{d,s} are ambiguous
when the input operand is in memory and no broadcast is being used.
While in Intel mode this gets resolved by printing suitable operand
size modifiers, AT&T mode need mnemonic suffixes to be added.

Properly display extra data/address size prefixes

X86 disassembler checks data and address size prefixes when displaying
instruction mnemonic and operands. For the extra data and address size
prefixes, their names depend only on the address mode, not the data and
address size prefixes. This patch changes x86 disassembler not to check
the data and address size prefix when printing extra data and address size
prefixes.

Properly handle multiple opcode prefixes

This patch updates multiple opcode prefix processing:
1. Always print prefix together with bad opcode.
2. Since the last seen segment register prefix is active, we only print
the active segment register in the memory operand.
3. The 0xf2 and 0xf3 prefixes take precedence over the 0x66 prefix as the
opcode prefix. Also the last of the 0xf2 and 0xf3 prefixes wins.
4. Ignore invalid 0xf2/0xf3 prefixes if they aren't mandatory.
