For now, this just protects against programming errors like having the
same drive back multiple non-qdev devices, or untimely bdrv_delete().
Later commits will add other interesting uses.
While there, rename BlockDriverState member peer to dev, bdrv_attach()
to bdrv_attach_dev(), bdrv_detach() to bdrv_detach_dev(), and
bdrv_get_attached() to bdrv_get_attached_dev().

cfi02 is annoying in that is ignores some address bits; we probably
want explicit support in the memory API for that.
In order to get the correct opaque into the MemoryRegion object, the
allocation scheme is changed so that the flash emulation code allocates
memory, instead of the caller. This clears a FIXME in the flash code.

Commit 5145b3d1cc revealed a bug in the lazy ROMD switch-back logic, but
resolved it by breaking that feature. This approach addresses the issue
by switching back to ROMD after a certain amount of read accesses
without further unlock sequences.

When checking pfl->rom_mode for when to lazily reenter ROMD mode,
the value was check was the opposite of what it should have been.
This prevent the part from returning to ROMD mode after a write
was made to the CFI rom region.

This was done with:

after checking that get_clock and new_timer never occur twice
on the same line. There were no missed occurrences; however, even
if there had been, they would have been caught by the compiler.
There was exactly one false positive in qemu_run_timers:
- current_time = qemu_get_clock (clock);
+ current_time = qemu_get_clock_ns (clock);
which is of course not in this patch.

As stated before, devices can be little, big or native endian. The
target endianness is not of their concern, so we need to push things
down a level.
This patch adds a parameter to cpu_register_io_memory that allows a
device to choose its endianness. For now, all devices simply choose
native endian, because that's the same behavior as before.

In the very least, a change like this requires discussion on the list.
The naming convention is goofy and it causes a massive merge problem. Something
like this _must_ be presented on the list first so people can provide input
and cope with it.

If a flash file of size smaller than the flash size is specified in
the -pflash option, the block driver returns error. But the
pflash_cfi0x ignores the error. This results in a flash content of all
zeroes. And the simulation aborts while executing code.
This patch adds the checks for errors from bdrv_read and escalates it
to the calling code.

The parameter is always zero except when registering the three internal
io regions (ROM, unassigned, notdirty). Remove the parameter to reduce
the API's power, thus facilitating future change.

The IO index is now stored in its own field, instead of being wedged
into the vaddr field. This eliminates the ROMD and watchpoint host
pointer weirdness. The IO index space is expanded by 1 bit, and
several additional bits are made available in the TLB vaddr field.
