Use a standard heading format for the index.rst file in a directory.
Using overlines makes it clear that individual documents can use e.g.
=== for chapter titles and --- for section titles, as suggested in the
Linux kernel guidelines[1]. They could do it anyway, because documents
included in a toctree are parsed separately and therefore are not tied
to the same conventions for headings. However, keeping some consistency is
useful since sometimes files are included from multiple places.

We merged our previous multiple-manual setup into a single Sphinx
manual, but we left some text in the various index.rst lines that
still calls the top level subsections separate 'manuals'. Update
them to talk about "this section of the manual" instead, and remove
now-obsolete comments about how the index.rst files are the "top
level page for the 'foo' manual".

The default "alabaster" sphinx theme has a couple shortcomings:
- the navbar moves along the page
- the search bar is not always at the same place
- it lacks some contrast and colours
The "rtd" theme from readthedocs.org is a popular third party theme used
notably by the kernel, with a custom style sheet. I like it better,
perhaps others do too. It also simplifies the "Edit on Gitlab" links.
Tweak a bit the custom theme to match qemu.org style, use the
QEMU logo, and favicon etc.

When we first converted our documentation to Sphinx, we split it into
multiple manuals (system, interop, tools, etc), which are all built
separately. The primary driver for this was wanting to be able to
avoid shipping the 'devel' manual to end-users. However, this is
working against the grain of the way Sphinx wants to be used and
causes some annoyances:
* Cross-references between documents become much harder or
possibly impossible
* There is no single index to the whole documentation
* Within one manual there's no links or table-of-contents info
that lets you easily navigate to the others
* The devel manual doesn't get published on the QEMU website
(it would be nice to able to refer to it there)
Merely hiding our developer documentation from end users seems like
it's not enough benefit for these costs. Combine all the
documentation into a single manual (the same way that the readthedocs
site builds it) and install the whole thing. The previous manual
divisions remain as the new top level sections in the manual.
* The per-manual conf.py files are no longer needed
* The man_pages[] specifications previously in each per-manual
conf.py move to the top level conf.py
* docs/meson.build logic is simplified as we now only need to run
Sphinx once for the HTML and then once for the manpages5B
* The old index.html.in that produced the top-level page with
links to each manual is no longer needed
Unfortunately this means that we now have to build the HTML
documentation into docs/manual in the build tree rather than directly
into docs/; otherwise it is too awkward to ensure we install only the
built manual and not also the dependency info, stamp file, etc. The
manual still ends up in the same place in the final installed
directory, but anybody who was consulting documentation from within
the build tree will have to adjust where they're looking.

Adds bit #4 to status/control field of CPU hotplug MMIO interface.
New bit will be used OSPM to mark CPUs as pending for removal by firmware,
when it calls _EJ0 method on CPU device node. Later on, when firmware
sees this bit set, it will perform CPU eject which will clear bit #4
as well.

When starting an L2 KVM guest with `ic-mode=dual,kernel-irqchip=on`,
QEMU fails with:
KVM is too old to support ic-mode=dual,kernel-irqchip=on
This error message was introduced to detect older KVM versions that
didn't allow destruction and re-creation of the XICS KVM device that
we do at reboot. But it is actually the same issue that we get with
nested guests : when running under pseries, KVM currently provides
a genuine XICS device (not the XICS-on-XIVE device that we get
under powernv) which doesn't support destruction/re-creation.
This will eventually be fixed in KVM but in the meantime, update
the error message and documentation to mention the nested case.
While here, mention that in "No XIVE support in KVM" section that
this can also happen with "guest OSes supporting XIVE" since
we check this at init time before starting the guest.

This patch adds a new documentation file, ppc-spapr-numa.rst,
informing what developers and user can expect of the NUMA distance
support for the pseries machine, up to QEMU 5.1.
In the (hopefully soon) future, when we rework the NUMA mechanics
of the pseries machine to at least attempt to contemplate user
choice, this doc will be extended to inform about the new
support.

TPM subsytem is split into backends (see commit f4ede81eed)
and frontends (see i.e. 3676bc69b3). Keep the emulated
hardware 'frontends' under hw/tpm/, but move the backends
in the backends/tpm/ directory.

This adds support for nvdimm hotplug events through GED
and enables nvdimm for the arm/virt. Now Guests with ACPI
can have both cold and hot plug of nvdimms.
Hot removal functionality is not yet supported.

The company 'Arm' went through a rebranding some years back
involving a recapitalization from 'ARM' to 'Arm'. As a result
our documentation is a bit inconsistent between the two forms.
It's not worth trying to update everywhere in QEMU, but it's
easy enough to make docs/ consistent.
Note that "ARMv8" and similar architecture names, and
older CPU names like "ARM926" still retain all-caps.

Implement support for TPM on ppc64 by implementing the vTPM CRQ interface
as a frontend. It can use the tpm_emulator driver backend with the external
swtpm.
The Linux vTPM driver for ppc64 works with this emulation.
This TPM emulator also handles the TPM 2 case.

Handle bit 1 write, then post event to monitor.
Suggested by Paolo, declear a new event, using GUEST_PANICKED could
cause upper layers to react by shutting down or rebooting the guest.

Add bit 1 for pvpanic. This bit means that guest hits a panic, but
guest wants to handle error by itself. Typical case: Linux guest runs
kdump in panic. It will help us to separate the abnormal reboot from
normal operation.

Firmware can enumerate present at boot APs by broadcasting wakeup IPI,
so that woken up secondary CPUs could register them-selves.
However in CPU hotplug case, it would need to know architecture
specific CPU IDs for possible and hotplugged CPUs so it could
prepare environment for and wake hotplugged AP.
Reuse and extend existing CPU hotplug interface to return architecture
specific ID for currently selected CPU in 2 registers:
- lower 32 bits in ACPI_CPU_CMD_DATA_OFFSET_RW
- upper 32 bits in ACPI_CPU_CMD_DATA2_OFFSET_R
On x86, firmware will use CPHP_GET_CPU_ID_CMD for fetching the APIC ID
when handling hotplug SMI.
Later, CPHP_GET_CPU_ID_CMD will be used on ARM to retrieve MPIDR,
which serves the similar to APIC ID purpose.

No functional change in practice, patch only aims to properly
document (in spec and code) intended usage of the reserved space.
The new field is to be used for 2 purposes:
- detection of modern CPU hotplug interface using
CPHP_GET_NEXT_CPU_WITH_EVENT_CMD command.
procedure will be described in follow up patch:

- for returning upper 32 bits of architecture specific CPU ID,
for new CPHP_GET_CPU_ID_CMD command added by follow up patch:

Change is backward compatible with 4.2 and older machines, as field was
unconditionally reserved and always returned 0x0 if modern CPU hotplug
interface was enabled.

'Command field' value is equal to 0, to reflect that currently it's not

Correct returned value description in case 'Command field' == 0x0,
it's not PXM but CPU selector value with pending event
In addition describe 0 blanket value in case of not supported
'Command field' value.

* Move reserved registers to the top of the section, so reader would be
aware of effects when reading registers description.
* State registers endianness explicitly at the beginning of the section
* Describe registers behavior in case of 'CPU selector' register contains
value that doesn't point to a possible CPU.
